JOB DESCRIPTION
TITLE: HOME HEALTH AIDE
RESPONSIBLE TO: RN SUPERVISOR
RESPONSIBLE FOR: The Home Health Aide is responsible for performing
tasks and providing care to assigned clients. All care is provided
in accordance with Greater Adirondack Home Aides policies and
procedures. Care provided in under the supervision of a Registered
Nurse and is described on the Care Plan.
All the duties and standards within this document will be performed
applying the missions and
values of Greater Adirondack Home Aides, Inc.; and in accordance
with the code of conduct,
established policies, procedures, and guidelines within the agency.
DUTIES:

RESPONSIBILITIES
| 1. Follows established policies and procedures of Greater Adirondack
Home Aides |
| 2. Complete Health Assessment per schedule (annually) |
| 3. Obtains PPD as scheduled |
| 4. Participates in and completes required inservice credit hours,
including mandatory
inservices, annually |
| 5. Attends and participates in employee meetings/opportunities as
necessary or requested |
| 6. Knowledge of/participates in Quality Improvement Program |
| 7. Other functions as may be required by Executive Director or
Designee |
QUALIFICATIONS:
The physical demands described are representative of those that must
be met by an employee to successfully perform the essential
functions of the job. Reasonable accommodations may be made to

- Home Health Aide
certification by a NYS Department of Health approved
program.

- Ability to read,
write, speak and understand the English language at the
8th grade level.

- Ability to work
under supervision and to follow a written care plan.

independently with flexibility and personal integrity.

effectively with patients, families, co-workers and
personnel from support agencies.
