JOB DESCRIPTION
TITLE: HOME HEALTH AIDE
RESPONSIBLE TO: RN SUPERVISOR
RESPONSIBLE FOR: The Home Health Aide is responsible for performing tasks and providing care to assigned clients. All care is provided in accordance with Greater Adirondack Home Aides policies and procedures. Care provided in under the supervision of a Registered Nurse and is described on the Care Plan.

All the duties and standards within this document will be performed applying the missions and
values of Greater Adirondack Home Aides, Inc.; and in accordance with the code of conduct,
established policies, procedures, and guidelines within the agency.

DUTIES:

A. PERSONAL CARE FUNCTIONS
 
1. Bathing Clients:
2. Skin Care
2a. Special Skin Care
3. Grooming
4. Toileting
5. Ambulation
6. Transferring, positioning, ROM
7. Using Medical Supplies/Equipment/Devices
8. Well Baby
9. Assisting with changing a clean dressing of a stable wound
9a. Assist with changing of dressings involving sterile procedure
10. Caring for a Mature/Stable Ostomy
11. Assist with self-administration of medications to self-directing clients
12. Simple Measurements and Tests

B. NUTRITIONAL SUPPORT

C. ENVIRONMENTAL SUPPORT

D. REPORTING AND DOCUMENTATION

E. EMPLOYEE RESPONSIBILITIES

1. Follows established policies and procedures of Greater Adirondack Home Aides
2. Complete Health Assessment per schedule (annually)
3. Obtains PPD as scheduled
4. Participates in and completes required inservice credit hours, including mandatory inservices, annually
5. Attends and participates in employee meetings/opportunities as necessary or requested
6. Knowledge of/participates in Quality Improvement Program
7. Other functions as may be required by Executive Director or Designee


QUALIFICATIONS:

  • Home Health Aide certification by a NYS Department of Health approved program.
  • Ability to read, write, speak and understand the English language at the 8th grade level.
  • Ability to work under supervision and to follow a written care plan.
  • Ability to cope with the mental and emotional stress of the position.
  • Ability to function independently with flexibility and personal integrity.
  • Ability to work effectively with patients, families, co-workers and personnel from support agencies.
  • Must be in good general health and demonstrate emotional stability.
  • Ability to relate to and work with ill, elderly or disabled persons.
  • Ability to lift a minimum of 50 pounds unassisted* frequently.
  • Ability to lift 50-100 lbs. unassisted occasionally.
  • Ability to push or pull frequently.
